Job Summary and Scope

 

The Ambulatory Care Pharmacist ensures the safe and effective use of medication by providing medication refill and ambulatory medication therapy management services ensuring optimal drug product selection, dosage regimen design, effective administration and drug therapy monitoring services at SRMC. The pharmacist monitors outpatient drug therapies and works with the medical staff to find alternatives when needed. The Ambulatory Care Pharmacist works to improve continuity of care by facilitating communication between the primary healthcare provider and the patient. This staff member works closely within a multidisciplinary team to ensure the best care is provided. The pharmacist supervises the pharmacy technicians and interns in the performance of their duties. This position is responsible for providing the best customer service to both internal and external customers. The pharmacist ensures adherence to state and federal regulations as well as hospital and departmental policies and procedures. The pharmacist will display a commitment to the hospital mission statement and role model the SRMC Values and consistently demonstrate in thought and action the SRMC commitment to “putting you first”.

Essential Functions: 

  • POLICIES AND PROCEDURES - Maintain established departmental policies and procedures, objectives, and quality assurance programs
  • PROFESSIONAL DEVELOPMENT - Enhance professional growth and development through participation in educational programs, reading current literature, attending in-services, meetings and workshops.
  • Provides direct patient care by utilizing NM Board of Pharmacy and Board of Medicine approved protocol.
  • Performs physical examinations of patients according to policy and procedure. Uses findings and clinical judgment to manage a disease state in accordance with protocol and develop an appropriate treatment plan based on findings.
  • Performs comprehensive medication reviews and addresses therapy adherence issues with patients and appropriately adjusts medications based on clinical findings.
  • Actively identify, mitigate, prevent, and report potential and actual drug-related problems or adverse drug events. Recommend rational therapeutic alternatives as needed to prevent future problems.
  • Interprets, evaluates and accurately processes medication requests in a timely fashion. Prior to authorizing prescriptions; screens for drug allergies, interactions, and appropriateness of medication, dose, frequency, and route of administration.
  • Performs pharmaceutical care duties including (but not limited to) anticoagulation dosing and monitoring, and chronic disease state management by collaborating with providers to assess and monitor patient progress.
  • Writes orders for prescription drugs and medical equipment and authorizes refills for medications.
  • Act as a healthcare team member by actively participating in medical team(s) and department meetings.
  • Develops clinical pathways and protocols for use in clinics.
  • Provides supervision for staff working in the ambulatory care clinic including pharmacy technicians, interns and prn pharmacists.
  • Provides patient education and medication counseling. Educates hospital and medical staff both individually and collectively.
  • Maintain accurate records of encounters. Responsible for meeting all state, federal and regulatory requirements associated with the Pharmacist Clinician and the Ambulatory Care Service.
  • Collects data and supervises inspections for quality assurance and safety purposes. Maintains all logs and necessary paperwork associated with regulatory and quality assurance requirements. Actively participates in LEAN processes and quality/safety improvement initiatives.
  • Practices financial stewardship by ensuring the most cost effective delivery of pharmaceutical care and medications.
  • Plans, develops and implements cost saving initiatives that affect drug costs and healthcare professional time utilization while maintaining the highest level of pharmaceutical care.
  • Provides education to healthcare professionals and students in accordance with SRMC’s mission statement.
  • Perform miscellaneous job-related duties as assigned by the Director of Pharmacy.

Required Qualifications

 

  • Current State of New Mexico Pharmacist License without limitations or restrictions
  • Pharmacist Clinician License or within 6 months of hire.
  • BLS certification within 90 days of hire.
  • ACLS certification within 90 days of hire.

Education/Experience

Education: BS in Pharmacy or PharmD

 

Work Experience: 2 years of related clinical experience.

 

Preferred Educational/Experience Requirements:

 

  • PharmD degree
  • Residency in an acute care setting or
  • Residency in ambulatory care
