RN/Total Joint Program Coordinator

Department: Surgical Specialty Clinics
Location: Rio Rancho, NM

Job Summary and Scope

Provide clinical support and education to elective surgical patients for the assigned program, and identify candidates for surgery. The program includes meeting with patients in clinic, working with surgeons and other staff at multiple health sciences campuses, assisting patients and family members with the surgery and recovery process; triage; preoperative screening and counseling sessions; clinical pathways and proactive coordinated planning for discharge and follow up care. The program is geared toward increasing patient satisfaction and patient outcomes, targeting zero surgical infection rates, evaluating patients who need inpatient rehab after discharge, reducing length of stay, and increasing inpatient/outpatient volumes.

 

Promote and restore patients' health through direct nursing care to assigned patients within the nursing process; collaborate with physicians and multidisciplinary team members. Provide clinical leadership for other staff and assume relief charge duties when designated and serve as a clinical resource for staff. Serve as a role model to promote a positive work environment and quality patient care while utilizing SRMC Values. Patient care assignment may include adolescent, adult and geriatric age groups.

The RN Program Coordinator collaborates with physicians and interdisciplinary team members in the internal and external operations to meet patient and family needs, develop guidelines, provide education, implement necessary research, monitor patient outcomes, suggest and implement successful initiatives to improve quality and grow volume in the program while marketing the product line.

Essential Functions:

  • POLICIES, PROCEDURES AND GUIDELINES- Maintain established hospital as well as departmental policies and procedures, objectives, and quality assurance programs

  • PROFESSIONAL DEVELOPMENT - Enhance professional growth and development through participation in educational programs, reading current literature, attending in-services, meetings and workshops.

  • Report potential and actual patient safety, concerns, medical errors and or near misses in a timely manner thru the chain of command and document within the PSN system.

  • Establish a compassionate environment by providing emotional psychological and spiritual support to patient, friends and families.

  • Promote patient’s independence by establishing patient care goals; teaching patient, friends, identified ley caregiver and family to understand condition, medications, and self-care skills; answering questions.

  • Maintain a cooperative relationship among health care teams and resolve patient problems and needs by utilizing multidisciplinary team strategies.

  • Active participation in hospital strategic initiatives such as quality improvement and monitoring TJC compliance.

  • Assist physician and coordinate diagnostic procedures for pre-surgery and post-surgery needs.

  • Assist in specialty clinic; support triage for selection of surgical candidates.

  • Seeks opportunities to improve patient safety through questioning of current policies and procedures.

  • Provide and document nursing care, and ensure that physician's orders are carried out.

  • Serve as liaison between patient/family and various members of the health care team, including internal and external members, and multiple system campuses.

  • Counsel and teach patients and families regarding medical conditions, diagnostic procedures, physical therapy, medications, pain control, and other interventions.

  • Encourage patients to actively participate in their own care by asking questions, and reporting treatment or situations that they don’t understand.

  • Proactively coordinate and communicate with various departments (both internal and external) to coordinate services, resolve operational issues, and improve quality of patient care.

  • Continuously reviews processes for operational improvement and efficiencies.

  • Track progress of the program and identify areas that require attention.

  • Support statistical data collection and report to leadership and designated committees.

  • Participate in programs and committees related to the specialty or as directed.

  • Identify and report/correct environmental conditions and/or situations that may put a patient at undue risk.

  • Perform miscellaneous job-related duties as assigned to support the program mission.

Accountability

Required Qualifications

  • Ability to interact and cooperate with co-workers (Teamwork), strong interpersonal communication

  • Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction. Superior interpersonal and communications skills with decisive leadership with above-average judgment.

  • Knowledge of principles, methods, and procedures for diagnosis, treatment, and rehabilitation of physical and mental dysfunctions, and for counseling and guidance

  • Knowledge of administrative and clerical procedures and systems such as word processing, managing files and records, designing forms, and other office procedures and terminology

  • Effective organizational skills required

  • Extensive interpersonal and problem solving skills Interpersonal/organizational skills to effectively coordinate staff and workflow in a busy surgery practice/program with the ability to effectively sustain multiple simultaneous tasks. Attention to detail and accuracy.

  • Passion for quality patient care

Education/Experience

Education: Bachelor degree in Nursing (nationally accredited), or must obtain within 12 consecutive calendar months of position.

 

Licenses/Certifications:

  • New Mexico license required.

  • CPR Certification for Healthcare Providers or for Professional Rescuers, or must obtain within 30 calendar days of date of position.

     

    Work Experience: Three (3) years clinical work experience

     

    Preferred Educational/Experience Requirements: Master’s Degree in Nursing, AAACN certification preferred or a certification in current specialty; Three plus (3+) years Outpatient Clinic experience in relevant specialty.

     

    Language: Bilingual / English-Spanish preferred.

Supervision

  • Job is supervisory to the extent that daily work direction is provided to personnel in subordinate classifications.
